What is the health benefits annual information request form?
A form sent out by our office yearly. The form should be filled out and returned to our office. This form is intended to update all information for anyone covered on our plan. This form is NOT an enrollment or change form.
What is the benefit of using a mail order pharmacy?
The benefit is that your prescriptions will be mailed directly to your residence. The deductible is waived on all mail orders and your prescription will not need to be filled as often.
Why do I need to keep my EOB?
EOB's can not be reproduced in our office because they are generated by an outside office. Please review all of the information on the EOB for errors and to also submit to your flex account.
When is open enrollment for flex?
Open enrollment for flex runs the entire month of November. To sign up for flex, please contact human resources and fill out the necessary paperwork. Forms need to be returned to human resources no later than November 30. A new form will need to be filled out every November and submitted in order for flex to go into effect January 1.
When is open enrollment for health benefits?
Open enrollment runs the entire month of May. In order to make an additions or changes to your plan, a new form must be filled out and submitted to the risk and benefits office no later than May 31. These changes will go into effect July 1, which is the start of the new fiscal year.
When do my benefits start over?
Your medical, dental, and vision benefits start over July 1 of every year.
